- Clean, service and provide a wide variety of hearing aid options
- Nexus letters (fees apply)
- Referrals for Speech Therapy and Vestibular Testing
- FMLA paperwork upon request (fees apply)
- Treat patients with worker’s compensation claims
8. For a seamless check-in process, please bring the following:
- Photo Identification- a driver’s license or passport are acceptable forms of identification and will be required at check in
- Proof of insurance coverage-physical or digital insurance cards or a letter showing proof of insurance will be required at check in
- A current list of medications, including dosage and frequency

10. What if I’m uninsured?
Insurance is not required to receive health care. We welcome patients who do not have insurance coverage. Our team is happy to discuss available self-pay options and next steps with you prior to your visit.
11.) How can I verify my insurance coverage?
As a courtesy, our office provides insurance verification of benefits. However, ultimately it is the patient’s responsibility to ensure that we participate in their insurance plan, verify their own benefits, confirm co-pay, co-insurance and any deductible payments toward coverage if applicable.
